Send Medical Records
Once we receive your completed patient intake form, we will contact you with steps on how to send relevant medical records, including copies of any radiology scans and test results. Detailed information about your medical condition will expedite your inquiry and our ability to schedule you with the correct specialist(s).

Obtain Medical Acceptance
Dr. Gwalani may review your medical history to determine if he is able to help your current pain condition. If so, your own personal patient coordinator will reach out with additional information.

Obtain Financial Clearance
You will need to provide either a deposit of estimated charges in advance or your insurance company must provide authorization approved by Mission Pain Treatment Center that guarantees payment.

Schedule an Appointment
An international patient coordinator will help you schedule an appointment at a convenient time with Dr. Gwalani. Your coordinator will help you complete all the necessary forms for appointment scheduling and registration.
